Keep ONA up-to-date with address changes and name change? It does cost the union to send mail to incorrect addresses. It then becomes difficult to keep those members informed of important news and events (For information on what to do)

Please remember to submit your new change of address or name to our Local Office. (We will forward to ONA Central for you).

Forms are available from your Nursing Rep, Site VP or the Local Office. Or you may down load a form and fax it to us at (613) 731-1307.
